简介:Mb是一种氧结合血红素蛋白,分子量17,800道尔顿,主要分布于心肌和骨骼肌组织,在急性心肌梗塞(AMI)时,心肌细胞坏死,Mb稀放入血液中。在症状出现约2~3小时后,血中Mb可超出正常上限,9~12小时达到峰值,24~36小时后恢复正常。Mb阴性有助于除外心肌梗塞。

Myoglobin
Myoglobin and hemoglobin are hemeproteins whose physiological importance is principally related to their ability to bind molecular oxygen. Myoglobin is a monomeric heme protein found mainly in muscle tissue where it serves as an intracellular storage site for oxygen. During periods of oxygen deprivation oxymyoglobin releases its bound oxygen which is then used for metabolic purposes.
The tertiary structure of myoglobin is that of a typical water soluble globular protein. Its secondary structure is unusual in that it contains a very high proportion (75%) of a-helical secondary structure. A myoglobin polypeptide is comprised of 8 separate right handed a-helices, designated A through H, that are connected by short non helical regions. Amino acid R-groups packed into the interior of the molecule are predominantly hydrophobic in character while those exposed on the surface of the molecule are generally hydrophilic, thus making the molecule relatively water soluble.
Each myoglobin molecule contains one heme prosthetic group inserted into a hydrophobic cleft in the protein. Each heme residue contains one central coordinately bound iron atom that is normally in the Fe2+, or ferrous, oxidation state. The oxygen carried by hemeproteins is bound directly to the ferrous iron atom of the heme prosthetic group. Oxidation of the iron to the Fe3+, ferric, oxidation state renders the molecule incapable of normal oxygen binding. Hydrophobic interactions between the tetrapyrrole ring and hydrophobic amino acid R groups on the interior of the cleft in the protein strongly stabilize the heme protein conjugate. In addition a nitrogen atom from a histidine R group located above the plane of the heme ring is coordinated with the iron atom further stabilizing the interaction between the heme and the protein. In oxymyoglobin the remaining bonding site on the iron atom (the 6th coordinate position) is occupied by the oxygen, whose binding is stabilized by a second histidine residue.
Carbon monoxide also binds coordinately to heme iron atoms in a manner similar to that of oxygen, but the binding of carbon monoxide to heme is much stronger than that of oxygen. The preferential binding of carbon monoxide to heme iron is largely responsible for the asphyxiation that results from carbon monoxide poisoning.

myoglobin (mī'əglō'bĭn) , protein molecule isolated from the cells of vertebrate skeletal muscle that is both a structural and functional relative of hemoglobin, the oxygen-transport protein of the blood of higher animals. Myoglobin, which is composed of a single polypeptide chain of 153 amino acid residues, has the ability to store oxygen by binding it to an iron atom; iron is part of myoglobin's essential chemical composition. The complete amino acid sequence of myoglobin has been determined; it is a relatively small protein with a molecular weight of approximately 17,000 grams per mole. The distribution of myoglobin among the higher animals is a reflection of its physiological function. It is found abundantly in the tissues of diving mammals, e.g., the whale, the seal, and the dolphin. High concentrations of myoglobin in these animals presumably allows them to store sufficient oxygen to remain underwater for long periods. Myoglobin is found abundantly in man only in cardiac muscle, which, by virtue of its essential function, must possess the capacity for continued activity when environmental oxygen concentrations are low. Myoglobin has been investigated intensely and is the first protein molecule to have been completely described in terms of its three-dimensional geometry. This achievement won the British scientist John Kendrew a share in the 1962 Nobel Prize for Chemistry.

myoglobin,
a protein found in the muscle cells of animals. It functions as an oxygen-storage unit, providing oxygen to the working muscles. Diving mammals such as seals and whales are able to remain submerged for long periods because they have greater amounts of myoglobin in their muscles than other animals do.
There is a close chemical similarity between myoglobin and hemoglobin, the oxygen-binding protein of red blood cells. Both proteins contain a molecular constituent called heme, which enables them to combine reversibly with oxygen. The heme group, which contains iron, imparts a red-brown colour to the proteins. The bond between oxygen and hemoglobin is more complex than that between oxygen and myoglobin and accounts for the dual ability hemoglobin has to transport oxygen as well as to store it.
In contact with venous blood, oxygen combines more readily with myoglobin than it does with hemoglobin, favouring the transfer of oxygen from blood to muscle cells. Thus, the oxygen that the working muscle requires for the energy-producing biochemical reactions is provided.
Myoglobin has been obtained in pure crystalline form from many sources. It has a molecular weight of 16,700, about one-fourth that of hemoglobin. Though the heme portion of all myoglobins is the same, the protein portions vary considerably between species.
Myoglobin has been of great importance in the elucidation of protein structure. In 1962 a share of the Nobel Prize for Chemistry was awarded to John C. Kendrew for work, utilizing the technique of X-ray diffraction, that permitted construction of a three-dimensional model of crystalline sperm-whale myoglobin.

五、肌红蛋白的结构与功能
肌红蛋白(myoglobin)是第一个被确定的具有三级结构的蛋白质,主要生物学功能是结合氧,是哺乳动物肌肉中储氧的蛋白质。肌红蛋白是一个相对比较小的蛋白,只由一条多肽链组成,呈扁平的棱形,含有一个血红素辅基(heme prosthetic group)。分子大小为4.5X3.5X2.5nm。

脯氨酸羟基化酶proline hydroxylase 氨基酸合成细胞质赖氨酸羟基化酶1ysine hydroxylase 氨基酸合成细胞质铁蛋白Teritin 储铁血液及胞间隙血铁黄素hemosiderin 储铁骨髓脱铁铁蛋白apoferritin 铁吸收肠粘膜运铁蛋白transferrin 运铁血液及胞间隙血红素接合蛋白hemopexin 回收分解的血红素铁储存性铁蛋白肝珠蛋白heptoglobin 回收分解的血红素(3),(4),(5),(6),(7)人体总铁的 60—70%存在于血红蛋白中,5%存在于肌红蛋白,10%存在于各种酶,不足 0.1%的铁存在于运铁蛋白中,20—30%存在储铁蛋白中。表 2 示出存在于不同铁蛋白中的铁量。表 2 成人铁在不同铁蛋白中的分布[7]男性 70Kg Hb16g/dl 男性 100Kg Hbl8g/dl 女性 45Kg Hb 12g/dl 功能性铁Hb Fe 酶及肌红蛋白 Fe 2.67g 0.45g 4.2g 0.64g 1. 26g 0.29g 储藏性铁运铁蛋白 Fe 储铁蛋白 Fe 0. 005g 0.5—1. 5g 0. 007g 0.5—1.5g 0. 003g 0.3—1.0g总量最高>6g 最低<2g 血红蛋白是人体中铁的主要存在形式,1956 年,Ingran 等建立血红蛋白的三维分子结构及与铁结合的分子模式。其分子量约为 65000 道尔顿,其中叶琳结构是结合铁的部位,血红蛋白含铁量 4 良低,为 0.35%。婴儿出生时,血红蛋白含量较高,为 18—19g/dl,随后很快下降,3—4 个月时,为 12g/dl,这一水平持续至 1 岁后开始缓慢上升至青春期时,男性出现明显升高至 13—17g/dl,并一直维持这一水平[6,7]。女性青春期时并未出现明显的上升。缺铁会直接引起血红素合成障碍。进而影响血红细胞的合成。储铁蛋白在婴儿出生时,中位数为 101ng/ml,1 个月后升至 356ndml,在 6 个月至 15岁时降至 30nyml(7。142nyml)。成人男女区别较大,女性为 30ng/ml,而男性为 140ng/ml。缺铁或铁摄入量过高会引起储铁蛋白合量的明显变化。一项缺铁儿童储铁蛋白合量研究表明,贫血儿童储铁蛋白合量低于 9ng/ml。而一项铁摄入过量研究显示,其含量可高达2930ng/ml。储铁蛋白铁合量为 20%[6,7]。其他合铁蛋白及其受体的研究也很多,但有关其随营养水平及年龄变化的情况却报导很少。运铁蛋白水平也常作为判断铁缺乏的标准,但体内以运铁蛋白形式存在的铁较少。机体中铁的水平除受到年龄、性别、健康状况的影响,还受到许多其他营养素的影响,如 VA、Zn、Ca、Mg、P 等营养素均对铁的吸收有一定影响。由此可见,尽管机体中总的铁含量并不高,铁也被归为微量营养素,但其在体内的存在方式却十分集中,主要以血红蛋白及储铁

形式存在,就局部需求而言,其与大量营养素相似[8]。铁在地球土壤及地壳中含量极为丰富,其储有量仅次于氧、硅、铝,排在第四位,铁资源如此丰富,其满足生物利用似乎不应成为问题。人体中的总铁含量很低,又身体中的铁的相当一部分还可被机体重复利用,这样成人每日净铁吸收量达到 1。2mg 即可满足生理需求。造成全球铁营养缺乏的根源是铁的生物利用率问题。不仅仅是人体,其他生物对铁的利用率也很低,因为自然界中天然存在的可溶性的铁资源相对十分贫乏。另外铁的毒性也可能是造成生物铁利用策略的原因,相对于铁的毒性,轻度缺铁引起的损害要微小的多。参考文献:1. 陈永存主编(1984) 应用营养学:矿物质缺乏 人民卫生出版社 P166-179 2. 武汉医学院主编(1983) 营养与食品卫生学:铁 人民卫生出版社 P29—30 3. 塌方伤是指包括塌方、工矿意外事故或房屋倒塌后伤员被掩埋或被落下的物件压迫之后的外伤,除易发生多发伤和骨折外,尤其要提出挤压综合症问题,肢体和肌内丰富的部位长期受压,组织血供受损,缺血缺氧,易引起坏死。肌肉坏死对全身影响极为明显,横纹肌分解释出的肌红蛋白进入血流通过肾脏排出体外。
一旦伤员从塌方中救出,压迫解除,血流恢复,上述的肌红蛋白大量经血循环流至肾脏,由于长期缺血缺氧,有酸中毒存在,肌红蛋白在酸性尿中大量沉积在肾小管,引起急性肾功能衰竭,这一全过程是挤压综合症的致死原因,故在抢救多发伤的同时,要防止急性肾功能衰竭的发生,如给碳酸氢钠、速尿和甘露醇以碱化尿液和利尿,不使肌红蛋白沉积而迅速随尿液排出体外。
急救措施
从塌方中救出,必须急送医院抢救,方可及时采取防治肾功能衰竭的措施。
